Der Antietam Creek ist ein Nebenfluss des Potomac River im sogenannten Hagerstown Valley im Osten der Vereinigten Staaten. Er entsteht im Franklin County im US-Bundesstaat Pennsylvania etwa vier Kilometer südlich von Waynesboro in Pennsylvania, wo sein East Fork mit dem West Fork zusammenfließt. Nach etwa achthundert Meter erreicht er das Washington County im US-Bundesstaat Maryland. Er mündet nach 66 km bei Antietam oberhalb von Harpers Ferry in den Potomac River.

Das Einzugsgebiet des Flusses umfasst 241 km² im Franklin County, Pennsylvania und im Washington County, Maryland.

Traurige Berühmtheit erlangte der Fluss durch die Schlacht am Antietam.
