Ein Testergebnis ist falsch negativ, wenn es fälschlicherweise anzeigt, dass das Testkriterium nicht erfüllt (also negativ) sei. Das Kriterium ist also tatsächlich erfüllt, wird aber vom Test nicht erkannt. Für ein falsch negatives Ergebnis wird häufig die englische Bezeichnung „false negative“ verwendet.

Bei einem statistischen Test mit Nullhypothese führt ein falsch negatives Ergebnis zu einem Fehler 2. Art.

Beispiel 1

Das Ergebnis eines Schwangerschaftstests ist falsch negativ, wenn er keine Schwangerschaft anzeigt, obwohl eine Schwangerschaft vorliegt.

Beispiel 2

Das Beispiel dient vor allem dazu, die Aussagekraft von negativen und positiven Testergebnissen zu vergleichen. Im Folgenden werden die Auswirkungen von falsch negativen Ergebnissen beschrieben. Die entsprechende Beschreibung von falsch positiven finden sich dort.

Eine Krankheit hat den Grundanteil 100 von 10000, das heißt im Schnitt sind 100 von 10000 Personen erkrankt. Ein medizinischer Test soll das Vorhandensein der Krankheit feststellen (positives Testergebnis), kommt aber in einem Prozent der Fälle zum falschen Schluss. Diese Situation kann mittels des folgenden Entscheidungsbaums dargestellt werden:

Das falsch negative Testergebnis ist hier rot hervorgehoben. Es bedeutet, dass durch die Fehlerrate von 1% einer von 100 Tests ein falsch negatives Ergebnis ergibt: Von den 100 kranken Testpersonen erhält eine die falsche Diagnose, gesund zu sein.

Durch den geringen Krankenanteil werden durch den Test 9.802 von 10.000 als gesund diagnostiziert. Davon sind fast alle (nämlich 9.801) auch tatsächlich gesund. Obwohl der Test eine Sensitivität und eine Spezifität von jeweils 99 % hat, hat die Diagnose gesund eine Sicherheit von 9.801/9.802 = 99,989798 % (die Segreganz des Tests): Eine Person mit der Diagnose „gesund“ ist mit einer Wahrscheinlichkeit von 99,989798 % tatsächlich gesund.
